KARDIOXIRURGIK SHAROITDA PULSATIK VA NOPULSATIK PERFUZIYANING MIYA, BUYRAK VA MIOKARD PERFUZIYASIGA TA’SIRI

Kardioxirurgik jarayonlarda organlarning yetarli perfuziyasi bemor natijalari va jarrohlikdan keyingi asoratlar xavfi bilan bevosita bog‘liq. Pulsatik va nopulsatik perfuziya modellarining miyaga, buyraklarga va miokardga ta’siri klinik amaliyotda muhim mavzudir. Pulsatik perfuziya fiziologik ritmga yaqin qon oqimi orqali organlarga kislorod va oziq moddalar yetkazilishini optimallashtiradi, endotelial va miokard funktsiyasini qo‘llab-quvvatlaydi. Nopulsatik perfuziya esa doimiy qon oqimi bilan xarakterlanadi, ba’zi hollarda qisqa muddatli barqarorlikni ta’minlasa-da, uzoq muddatli perfuziyada organlarda regional qon yetishmovchiligi yuzaga kelishi mumkin.  Eksperimental va klinik tadqiqotlar shuni ko‘rsatadiki, pulsatik perfuziya miyadagi oksigen yetkazilishini yaxshilaydi, postoperativ kognitiv disfunktsiya xavfini kamaytiradi va buyrak funksiyasini saqlashga yordam beradi. Miokard perfuziyasi uchun pulsatik qon oqimi kislorod yetkazilishini optimallashtiradi, ishemiya va reperfuziya jarayonlaridagi shikastlanish xavfini kamaytiradi. Shu bilan birga, perfuziya strategiyasini tanlashda individual bemor xususiyatlari, jarrohlik davomiyligi va komorbid kasalliklar hisobga olinishi muhimdir.
