Morbid obesity is a global public health problem requiring a comprehensive therapeutic approach. Bariatric surgery remains the most effective method for long-term weight loss in patients with severe obesity. This paper highlights current aspects of perioperative patient management, including the diagnosis of comorbid conditions such as paraesophageal hernias, revision bariatric surgery strategies, and the role of reconstructive surgery in eliminating excess skin and fat after massive weight loss. Indications for abdominoplasty as the final stage of comprehensive treatment are discussed. An analysis of current approaches to microsurgical reconstruction of post-traumatic cicatricial contractures is presented. The need for a multidisciplinary approach to achieve optimal functional and aesthetic results is emphasized.

At present, the prevalence of type 2 diabetes mellitus (DM-2) is steadily increasing. Surgical methods of treatment (restriction and shunting operations) occupy a leading position in the treatment and correction of this pathology due to the highest metabolic efficiency. Each of the existing shunting operations (gastric gastrointestinal, biliopanсreatic bypass (PBS)), in turn, is used in various modifications. Among the operations of BPS are known modifications of Scopinaro, modification of Hess-Marceau and SADI appeared in recent years (duode-iliac bypass with gastric resection and one anastomosis).

In the period from 2022 to 2024. In the department of planned surgery of the city clinical hospital No. 1 (City Clinical Hospital No. 1. Tashkent), 40 patients were treated as inpatients whose BMI exceeded 30 kg/m2. Of the operated patients, 27 (67.5%) were women, 13 (32.5%) were men. The average age was 37.3 ± 0.84 years. Laparoscopic gastric bypass was performed in 8 (20%) patients, and 32 (80%) patients underwent “sleeve” gastrectomy.
